2008 Annual Community Awards Nomination Form

There are nine awards for which you can submit a nomination.

You may choose to submit your nominations in one or more categories. You do not have to submit a nomination in all nine categories.

Make sure to include detailed background information for the nomination in the "reasons" space provided - it is important that you detail how your nomination matches with the criteria, and provide examples. 

Please take the time to complete the nomination in full – remember that the selection committee does not always know the individual, business or organization being nominated and they require as much information as possible to make a fair selection.

Nominations can be made online on this page (scroll down), by fax at 250-334-4908 or delivered to the Chamber office at 2040 Cliffe Avenue.

Nominees are required to be a member of the community of Comox Valley; however, they are not required to be members of the Chamber of Commerce.

Recipients are chosen by how well their achievements match with the criteria, and are not based on the number of nominations submitted.

Nomination Deadline: Friday December 19, 2008 at 4:30pm.

The Awards will be presented Saturday January 31st at the Annual Community Awards Gala.  Hope to see you there!
