2009 Annual Community Awards Nomination Form

You will find the Nomination Form On Line just scroll down, or you may choose to print a Paper-Based Nomination Form.  Paper-Based nominations can be submitted by fax to 250-334-4908 or delivered to the Chamber office at 2040 Cliffe Avenue.

There are ten awards for which you can submit a nomination.

You may choose to submit your nominations in one or more categories. You do not have to submit a nomination in all ten categories.

Make sure to include detailed background information for the nomination in the "reasons" space provided - it is important that you detail how your nomination matches with the Award Criteria, and provide examples or your nomination may not be eligible for review by the committee.

Please take the time to complete the nomination in full – remember that the selection committee does not always know the individual, business or organization being nominated and they require as much information as possible to make a fair selection.

Nominees are required to be a member of the community of Comox Valley; however, they are not required to be members of the Chamber of Commerce.

Recipients are chosen by how well their achievements match with the criteria, and are not based on the number of nominations submitted.

A new Nomination Deadline has been set for Friday January 8th at 12:00 noon.

The Awards will be presented Saturday January 30th at the Annual Community Awards Presentation & Gala.  Watch for more information to come regarding the evenings events & ticket sales.
